How to Apply

STEP ONE

Download the Work Permit Application.

• MA Employment Permit Application

You, your employer, your parents, and possibly a doctor will each need to fill out sections of the application.

STEP TWO

Scan your completed application along with a copy of your license, student ID, or other personal identification. During the school year, please contact Ms. Coogan, and during the summer, reach out to Ms. Patterson.

STEP THREE

• Please be sure that you (the student) sign the completed form before presenting it to the employer.
• Once signed, you may present the permit to your employer, and a copy of the signed permit will be filed for MA State records.
