How to Apply
STEP ONE
Download the Work Permit Application.
You, your employer, your parents, and possibly a doctor will each need to fill out sections of the application.
STEP TWO

Please be sure that you (the student) sign the completed work permit application at the bottom of page 2. Once signed, present the application to your employer, and they should fill out and sign page 1.

Then on page 2, your doctor must sign the first part (if you are under the age of 16), and your parent/guardian must sign. Once all of this is completed, move on to Step Three.
STEP THREE
- Scan your completed application along with a copy of your license, student ID or other personal identification, and email it or bring these items into the Main Office at the High School and see Ms. Coogan or Ms. Patterson to obtain a Work Permit.
